By choosing this option, I affirm my dedication to Luna and acknowledge that Luna will continue to maintain my credentials and payer contracts, ensuring my ability to treat patients under the Luna name. Furthermore, I understand and accept my responsibility to keep my license, Certificate of Insurance (COI), and Basic Life Support (BLS) certification up to date and on file with Luna.
By selecting this option, I indicate my intention to remain affiliated with Luna. Although I may not be currently available to treat patients, I am committed to maintaining my relationship with Luna and will resume patient care when circumstances permit. Furthermore, I understand and accept my responsibility to keep my license, Certificate of Insurance (COI), and Basic Life Support (BLS) certification up to date and on file with Luna.
I understand that choosing this option will result in my removal from all payer contracts associated with Luna. If I decide to rejoin Luna in the future, I acknowledge that I will need to complete the onboarding and credentialing process from scratch. I will schedule a formal call with the team.
At Luna Physical Therapy, we are committed to providing the highest quality care to our patients while ensuring compliance with payer contracts and effectively planning for patient demand. To support these goals, we ask our inactive therapists who have not seen a patient in greater than 60 days to reattest their commitment to Luna every 60 days. This regular re-attestation process enables us to accurately forecast patient demand, plan staffing levels accordingly, and maintain compliance with payer contracts. By reaffirming your affiliation with Luna at regular intervals, our therapists help us better allocate resources and ensure continuity of care for our patients.
